Care2U is a medical practice and mobile health company that delivers healthcare directly to people in their home, on-demand, helping them avoid unnecessary and preventable hospitalizations and trips to the ER.
Care2U delivers healthcare directly to the patient, at home, in the community. Whether you need an on-demand acute care at home visit, Transitional care support, or ongoing Acute Care in the home, Care2U is here to support you. Our team brings all of the clinical capabilities of a traditional medical office or skilled care setting, and will streamline communication among all members of your care team.
Care2U offers a multidisciplinary medical team to support your care needs. Our mobile care team consisting of Care Specialists and RNs will provide care on-site in your home. Our team of ER Physicians, Hospitalists, and Nurse Practitioners lead and support care via telehealth and are available for in-person visits.
Upon arriving, our Care Technician will take vital signs, help assess your immediate needs, and conduct a comprehensive Medical History and Physical.
Our ER Physician will join the visit via telehealth to review your medical history, provide care recommendations During the visit, our Care Technician will perform additional diagnostic tests as recommended by the physician.
Care2U clinicians can treat non-life-threatening medical conditions, delivering most clinical capabilities of a traditional skilled care setting right to the patient’s home. Our team can treat UTis, dehydration, Pneumonia, Cellulitis, COPD, and CHF exacerbation, among others.
We can deliver IV Fluids, IV antibiotics, IV Lasix, conduct Point of Care Diagnostic Testing, perform X-rays, ultrasounds, EKGs, Echo, and perform wound care needs.
On average, our appointments typically last about 60 – 90 minutes.
After you request a Care2U appointment, our team will assign you our next available appointment. All of our appointments are scheduled with a two-hour arrival window to account for traffic and other factors. Our provider will arrive within that two-hour window but please note that the appointment may not be completed by the end of the arrival window..
Yes! You can book a Telehealth Consultation with one of our ER Physicians if you aren’t ready to have a clinician come into your home, or if you just have some questions you’d like answered.
Our team understands how stressful a transition home from the hospital or nursing facility can be. Care2U Transitional Care is here to help ensure that your discharge home and transition back into life in the community is as smooth as possible, and that you don’t end up back in a hospital. Our clinician will visit with you within 48 hours of arriving home, and you’ll have access to our clinical team in-person, over the phone, or via telehealth, should you need them.
We offer three convenient options for COVID-19 testing, including Rapid Antigen tests, with results in 15-20 minutes, Traditional PCR tests, with results in 24-48 hours, or Rapid PCR tests, with results in 4-6 hours. We bring the test directly to you!

Our mission is to ensure we always deliver high quality care to your home at an affordable price. Our Acute Care program, providing hospital-level care at home, is covered by most insurance plans. For our high complexity urgent care at home, there is a minimum out of pocket delivery fee of $179 to have a mobile field clinician come to your home. The remainder of the visit will be charged to insurance.
Yes. We currently partner with most leading insurance companies, including Medicare. If you’d like to confirm coverage under a particular plan, please call our office and a member of our team will be happy to help you.
