Job Summary: 

As a Medical Assistant, you will be an integral part of our healthcare team, collaborating with physicians, APPs, and clinical staff. Your responsibilities include assisting with daily rounds, facilitating referrals for clinical services, preparing necessary paperwork, and participating in Remote Patient Monitoring activities. The ideal candidate has excellent communication skills, exceptional interpersonal skills, and at least 1 year of experience in Adult medicine, Geriatrics or Adult Urgent Care. This role requires strong organizational skills, attention to detail, and proficiency in EMR Software, eClinicalWorks preferred.  

Responsibilities:

• Work closely with Physicians, APPs, and other clinical staff to ensure all interpreted results are clearly relayed and documented. 
• Assist with paperwork for Assisted Living Evaluations. 
• Perform chart QA/QI as directed by the supervisor. 
• Monitor and ensure vendor SLA compliance. 
• Contribute to daily care coordination rounds and provide high-quality written and verbal communication with physicians, field providers, patients, families, referral partners, care managers, and vendors. 
• Report testing results to telemedicine physicians on duty. 
• Ensure timely and complete follow-up of results with patients, Health Care Proxy, and/or referring parties. 
• Participate in Remote Patient Monitoring activities. 
• Facilitate referrals for Durable Medical Equipment, Skilled Nursing, Wound Care, Hospice, or Palliative Care, and other clinical services.  

 Required Skills/Abilities:

• At least 1 year experience as a Medical Assistant  
• Strong organizational and communication skills. 
• Ability to work in a high-paced and sometimes stressful environment. 
• Knowledge of medical terminology and procedures. 
• Familiarity with Microsoft Office Suite 
• Competency and comfortability working in an EMR, eClinicalWorks preferred
